2008-09 Test Dates and Registration Deadlines

     (Optional MELAB Speaking Test is not available in Minnesota.)

* Registration form and payment must be received by mail by the deadline date or your registration will be returned. Late registrations will not be accepted.
** Score reports for December test will take six weeks.

Registration

Print out and complete the MELAB Registration Form.
Attach a recent, passport-sized photo of yourself to the form.
Include the test fee of $80.00 by personal check or money order.
Mail your Official Identification Form, the photo, and the fee to:

PHOTO: One recent, original, passport-sized photo is required. The photo may be black/white or color. The photo must be sent in with your registration. Scanned or photocopied photos are not acceptable.

FEES: Test fee must be paid by a personal check or money order payable to "University of Minnesota." A fee of $20.00 will be charged for insufficient checks. NO REFUNDS WILL BE GIVEN. If you cancel your test or if you are absent on test day, you will not receive a refund.

CONFIRMATION: You will receive instructions by mail confirming your registration and giving you the location of the test and items to take to the test.
Identification Documents

You must bring your valid passport or Permanent Resident Card (green card). If you do not bring your passport or green card to the test, you will not be admitted and you will not receive a refund. (If your passport is expired or if you do not have either of these documents, you must phone our office. Do not send your registration until you have spoken to us. 612-626-8648)

If the photo on your document is an outdated photo of you as a child, you also must bring a second form of identification, either a valid driver's license or a school Photo I.D. card. Bring both passport and the second identification.

Score Reports

The official score reports will be sent to the mailing address listed on your registration form four weeks after the test date. (Note: scores for the test on December 20th will be sent six weeks after the test date.) Free reports are also sent to the one or two schools listed at the bottom of your registration form.

You may pay $5.00 each for extra reports sent to more schools. On test day, bring a check or money order payable to "English Language Institute" and an Additional Score Report Request Form, completed with the mailing address of the school.
